At Back Mountain Pediatric Dentistry, we understand that navigating care plans isn’t always fun. If you’re not sure whether you can use your insurance at our office, we encourage you to reach out to our team to do a complimentary benefit assessment for you!
Most insurance plans cover the cost of twice-yearly exams and cleanings. They also cover the cost of most preventive treatments such as fluoride treatments and sealants. Please note that no insurance company covers the cost of other services completely and that your insurance company determines the cost of services and percent coverage.
The cost of treatment depends on your child’s individual needs and treatment plan. We will provide an estimate of costs before any work is performed, but we cannot guarantee that estimates will be the same amount that your insurance company covers.
Your estimated out of pocket cost is due at the time your child’s treatment is completed. We will file your insurance claims as a courtesy to you. Once the insurance payment is received, you will be notified if there is any remaining balance or refund. As the subscriber, you will be responsible for any amount not covered, non-covered services, and/or expenses determined not allowed by your insurance company.
For your convenience, we accept cash, personal checks, Mastercard, Visa or Discover.
We recognize that under certain circumstances an account balance may be incurred. Back Mountain Pediatric Dentistry requires that all outstanding balances be paid within thirty (30) days of receiving your statement unless other arrangements have been made. Also, note that if we have not received payments or you have not contacted us within thirty (30) days, further action may be taken with a collection agency. We reserve the right to apply an interest rate of twenty (20) percent from the date of service.
We want to inform our valued patients that Back Mountain Pediatric Dentistry does not participate with state insurance programs such as GHP, Amerihealth, or CHIP at this time.
